    Normal Foot, High Arch, Low Arch

    Of congenital or acquired origin, a high arch can be considered the opposite condition to the so-called low arch-

    When present, the most common symptoms consist of pain in the feet, pain in the ankles, instability of the ankles, and problems on toes.

    For a correct diagnosis, physical examination and anamnesis are often enough.

    Depending on the severity of the symptomatic picture, the treatment can be conservative or surgical.

    Brief Reminder of What the Medial Arch is

    As you can see from the image below, the human feet have, on the inside edge, a raised area that detaches from the support on the ground. This raised area is called the medial plantar arch.

    The height of the medial arch – that is, how much the raised area detaches from the ground – varies from person to person.

    What is a High Arch?

    A high arch is an anatomical deformity, in the presence of which an individual’s feet have an excessively high plantar arch.

    The presence of a medial plantar arch higher than normal changes the support of the feet on the ground: if in people with feet without anomalies, the support affects the heel, the central external part and the front part (where the toes reside ), in people with a high arch, the support only affects the heel and the front area.

    This anomaly of the support on the ground redistributes the body weight. Instead of weighing on three parts (heel, external central part and front area), it focuses only on the areas that rest on the ground i.e., heel and front portion.

    It is important to specify that a high arch is a result not only of an increase in the internal anatomical structures that form the medial plantar arch. But also of an accentuated downward curvature of the anterior area of ​​the feet, particularly the area corresponding to the first toe (or big toe).

    In addition, many people with hollow feet present problems within the calf muscles and the tendons (NB: that important fibrous structure that connects the calf muscles to the heel, i.e., the bone that constitutes the heel).

    A high arch is an opposite condition to the low arch.

    With the terms “low arch,” doctors identify an anatomical malformation in which an individual’s feet have a lower than normal or completely absent medial plantar arch.

    In people with a low arch, therefore, the internal central part of the feet rests completely on the ground: this alters the distribution of body weight on the feet and predisposes them to be painful and degenerative phenomena of the joints, muscles, bones, and ligaments.

    Causes

    High arch can be:

    • A congenital condition, transmitted by one of the parents as a somatic characteristic;
    • An adaptive condition, subsequent to the presence of certain favoring factors;
    • An idiopathic condition. In medicine, the term idiopathic, associated with the name of a pathology, indicates that the latter arose for unknown or unidentifiable reasons.

    Furthermore, a high arch is a more common condition in the female population than in the male population, and according to Anglo-Saxon research, one in 5 hollow foot cases is idiopathic.

    Symptoms and Complications

    The presence of a high arch can be asymptomatic – therefore, do not involve any disturbance – or symptomatic.

    When the high arch is symptomatic, the symptoms may consist of:

    • Pain or discomfort in the feet, especially in the sides or in the metatarsal area;
    • Ankle pain;
    • Ankle instability. This predisposes to repeated sprains of the ankles;
    • Sense of rigidity and/or insensitivity to the feet or ankles;
    • Difficulty standing many hours in a row, walking long distances or running. In these situations, the difficulties are due to the painful sensation, which intensifies during these activities;
    • Presence of issues on toes;
    • Calluses appear in certain specific areas of the feet, such as the heel, the outer edges or the metatarsal area.

    Complications

    In some unfortunate circumstances, high arches can lead to complications.

    The most common complications are the so-called peroneal tendonitis (or peroneal tendonitis), Achilles tendon problems (e.g., rupture), plantar fasciitis, or the so-called ankle conflict syndrome.

    It is also important to remember that progressive neurological pathologies are often responsible for:

    • A growing worsening of the high arch;
    • The appearance of a particular painful sensation, known as neuropathic pain.

    Remedies: Orthotics, Sports Support Bands and Targeted Exercise

    The first thing to do in the presence of a suspected high arch is to notice it. It seems nonsense, but many parents and many family doctors do not “see” the problem and do not intervene immediately, that is, during the development phase (up to 6-7 years) when physiotherapy sessions can be extremely useful in correcting the defect.

    You can also intervene with targeted exercises (e.g., walking on special structures with controlled inclination, performing push exercises that stimulate the front part of the foot to “compensate” the curve). Finding the best work shoes for high arches is also another wonderful solution.

    The clogs most used by nurses are of three types, light in EVA, in hard rubber, and in wood and leather. All have the aim of being comfortable, having the right support, and of ensuring safety.
